Thulium Fiber Laser Setup

35W & 70W systems โ€” power, fibre connection & parameter calibration

Setup Checklist
  1. Position the laser console on a stable trolley within 1.5m of the operating table. Ensure ventilation clearance of at least 30cm on all sides.
  2. Connect the power cable to a dedicated 15A socket. Switch on the main power breaker at the rear of the unit.
  3. Insert the Thulium laser fibre (200μm or 365μm) into the fibre port. Rotate clockwise until the locking collar clicks. Perform a visual end-face check.
  4. Boot the system and allow the warm-up cycle to complete (approximately 2 minutes). Confirm the ready indicator light is green.
  5. Set the desired energy (0.2–3.0J), frequency (5–80Hz), and mode (Dusting / Fragmentation / Vapourisation) on the touchscreen panel.
  6. Perform a test pulse into a wet gauze pad at low power (5W) to confirm fibre integrity and beam output before connecting to the scope.
  7. Activate foot pedal and confirm laser fires on demand. System is now ready for the procedure.

Flexible Ureteroscope Setup

Light source, camera stack & irrigation connection

Setup Checklist
  1. Inspect the scope under white light for fibre bundle integrity. Confirm deflection tip moves freely in all directions with no resistance.
  2. Connect the light guide cable to the light source. Set brightness to 60–80% initially โ€” increase once inside the collecting system.
  3. Attach the camera head to the eyepiece (or digital port for chip-on-tip scopes). White balance and focus using the camera processor.
  4. Connect the irrigation inflow port to the pressure irrigation set. Prime the tubing and confirm bubble-free flow at 150–200 cmH₂O.
  5. Pass the laser fibre through the working channel. Confirm the fibre tip protrudes 3–5mm beyond the distal end before activating the laser.
  6. Test active deflection: confirm >270° in the primary plane and passive deflection in the secondary plane.
  7. Flush the working channel with saline. Scope is ready for ureter introduction via the ureteral access sheath.

Rigid Cystoscope Setup

Sheath assembly, optic insertion & irrigation priming

Setup Checklist
  1. Select the appropriate sheath size: 17Fr for diagnostic, 22Fr for operative procedures. Confirm the obturator slides smoothly within the sheath.
  2. Insert the telescope (0° for bladder neck / trigone, 30° for anterior wall, 70° for bladder dome) and lock the lens connector.
  3. Attach the light cable and camera head. White balance against a white swab and focus at 1cm distance.
  4. Connect irrigation inflow to the sheath stopcock. Attach the outflow tubing to the drain bag. Use isotonic saline at room temperature.
  5. Open the inflow stopcock and prime the system until fluid runs clear at the outflow tube with no air bubbles.
  6. Introduce the cystoscope with obturator in situ, under direct vision or with tactile guidance. Switch to the telescope once in the bladder.
  7. Adjust irrigation flow to maintain clear visualisation. Confirm all instrument ports are patent before beginning the procedure.

Nephroscope Setup

Percutaneous access, sheath mounting & irrigation circuit

Setup Checklist
  1. Assemble the nephroscope: attach the bridge to the outer sheath (24Fr or 26Fr). Confirm the inner working channel is clear and patent.
  2. Connect the light source cable and camera head. White balance and confirm clear optics. Adjust camera orientation for standard anatomical view.
  3. Attach the irrigation inflow line to the upper port of the bridge. Connect the outflow / suction line to the lower port or the Amplatz sheath side arm.
  4. Prime the irrigation circuit with saline. Set pressure to 80–120 cmH₂O for initial visualisation; reduce once access is confirmed to minimise intrarenal pressure.
  5. Pass the laser fibre through the working channel. Confirm 3–5mm protrusion beyond the telescope tip before firing.
  6. Ensure the stone retrieval basket and forceps are available at the instrument table for fragment extraction.
  7. Confirm inflow and outflow are balanced. Scope is ready for introduction through the Amplatz sheath after renal access is established.
